Information, Advice & Guidance Kingston borough residents of pension age, their families and informal carers Raleigh House Day Centre, New Malden People aged 65+, living in Kingston borough. The centre is able to welcome those living with dementia and who have personal care needs. Transport can be arranged for those who need it, subject to availability. The Bradbury Centre, Kingston People aged 65+, living in Kingston borough. The centre is suitable for people who can manage their personal care independently and make their own way to the centre. (We can support with Taxi-Card and Dial-a-Ride applications.) Older People's Community Team People resident in Kingston borough and/or registered with a Kingston GP. Home from Hospital Service People resident in Kingston borough and/or registered with a Kingston GP. Handyperson Older people and people with disablities living in Kingston borough. Help@Home Older people living in and around Kingston borough. Community Connectors Team (Social Prescribing Link Workers) Adults 18+ registered with one of the following GP surgeries: Central, Langley, Brunswick, Red Lion, Hook, Sunray, Chessington Park, Fairhill, Fairfield, Kingston Health Centre, St Albans.
